7/15/02: HEY, YOU CAN TYPE OUT ALL YOUR STUFF RIGHT INTO THIS FORM, IF YOU HAVE ACROBAT 4.0 OR BETTER, then print it, sign it, hand it in. It even uses a different font for your bits versus the form text. I didn't even know you could DO that with Acrobat, it's really cool :). Bad top cops everywhere will HATE it :).
HOW TO USE THIS FORM:
Complete sections 1 through 5, and turn it in at your Police Department or Sheriff's office.
Get a receipt from whoever you give it to :).
They are allowed to charge up to 20% of their local agency fee ($20, which is 20% of $100 which is the max) as a non-refundable application processing fee. They might want you to pay for fingerprinting; max fee is $25 or the normal fee they usually charge, whichever is lower. See also Penal Code 12054 and "Section 7" for info on fees.

The only reason a police department could legitimately give for refusing to take the application is if the town isn't incorporated (therefore they're not allowed to issue) or they're a "contract police agency" under the Sheriff's office or a neighboring town in which case they can send you to that other agency, or in cases where a town has "walked away from the CCW process" and dumped it (in it's entirety) on the Sheriff. Sheriffs MUST take your application.
